As of September 1, 2025, new rules for issuing sick leave certificates came into force in Russia, which particularly affect those who frequently take sick leave. The changes, approved by the Russian Ministry of Health, also apply to residents of the Nizhny Novgorod region.
Now, if an employee has taken sick leave four times in the past six months, on the fifth request they will receive a sick leave certificate for only three days. It can be extended only by decision of a medical commission composed of several specialists.
It is important to note that the new rules take into account all sick leave certificates regardless of the reasons for illness. This means that even with different diagnoses, the total number of sick leaves over six months will matter.
However, the changes will not affect all categories of patients. The restrictions do not apply to cases of caring for sick children and relatives, undergoing a course of medical rehabilitation, or treatment of severe and socially significant diseases. In particular, this includes illnesses requiring renal replacement therapy.
If a physician believes that a patient does not need a sick leave certificate but the patient insists on obtaining one, the decision will also be made by the medical commission. This rule also applies if it is necessary to extend an already issued sick leave beyond three days.
Financing of sick leaves exceeding the established three-day period will be provided by employers and the Social Insurance Fund only upon presentation of the commission's conclusion. The changes are aimed at tightening control and reducing the number of unwarranted sick leaves.
The new rules will be in effect until September 1, 2031.
It was previously reported that updated rules for providing medical care to foreign nationals came into effect on September 1.
