How to prevent tuberculosis from developing: two steps from a Nizhny Novgorod phthisiatrist
As reported by Natalia Borodina, deputy chief physician of the Nizhny Novgorod Regional Clinical Anti-Tuberculosis Dispensary, Russia continues to be among the countries with a high burden of tuberculosis — according to WHO estimates, up to 1 million people die from this disease worldwide each year. The main source of infection is a sick person who expels pathogens with sputum. When sneezing, coughing, laughing, or talking, particles of sputum containing tuberculosis bacteria spread in the air up to 15 meters and can penetrate the lungs of other people. The risk of disease increases with a decrease in immunity and protective functions of the body, for example, in chronic bronchitis in smokers. According to the specialist, the main form of the disease remains pulmonary tuberculosis: among newly diagnosed patients, it accounts for about 95%, while extrapulmonary forms account for just over 4%. At the same time, bone and joint tuberculosis is more common in men, while urinary system tuberculosis is more common in women. For diagnosis in suspected respiratory tuberculosis, the following are conducted: chest X-ray (overview X-ray in two projections, tomographic study); complete blood count; threefold sputum examination for acid-resistant mycobacteria by microscopy; tuberculin test (Mantoux reaction). "If indicated, the district doctor issues a referral for a full course of anti-tuberculosis therapy. After treatment, the patient is placed on dispensary registration for a minimum of 2 years," explained Natalia Borodina. The key method for detecting tuberculosis is fluorography: its widespread implementation has increased the effectiveness of disease detection by 2-3 times. For accurate diagnosis, modern methods of radiological and functional diagnostics are used, including CT, ultrasound, and endoscopic methods (for example, video thoracoscopy). Thanks to the combination of non-invasive and invasive methods, a diagnosis can be established in more than 95% of patients in the shortest possible time. The doctor noted that 80% of the population of the country are carriers of tuberculosis mycobacteria, but in 95% of cases, they remain in a "dormant" state. The main recommendations for prevention are: regularly undergo fluorographic examinations as part of medical check-ups; if chest pain, coughing with sputum, fever, or night sweats occur — immediately seek further examination. "One patient can infect up to 10 people in a year, which requires additional government spending on detection and treatment of the disease. Tuberculosis can be prevented and cured — Russian treatment methods are significantly better than in many countries, and the criteria for recovery are stricter," emphasized Natalia Borodina.
